Healthcare and Beyond: Enhancing Patient Care with Extended Care Teams
Traditional medical treatment has had patients rely solely on their Primary Care and/or Specialty Care providers for all their health concerns and issues. We know that healthcare is comprised of so much more including Behavioral Health, SDOH, Pharmaceutical and Nutrition. This conference will provide an opportunity to strengthen collaboration amongst various disciplines throughout the Enterprise, while also allowing our community partners to develop a better understanding of Extended Care Teams and how they enhance patient care. Please join us in taking a closer look at how AHN Extended Care Teams utilizes a team-based approach, working alongside the physician, to create holistic care for patients where better health outcomes can be achieved.
