8th Annual Immigrant/Refugee Conference: Empowering Immigrant and Refugee Health: The CHW Approach to Care Navigation

The 8th Annual Immigrant/Refugee Conference will focus on the subject of Community Health Workers (CHWs) role in connecting and navigating immigrant patients accessing care. CHWs play a crucial role in connecting immigrants to healthcare services and facilitating their navigation through the healthcare system. CHWs are vital in bridging the gap between immigrant communities and the healthcare system.  Their cultural understanding, language skills, and community ties make them uniquely positioned to connect immigrants with healthcare services, address barriers to access, and promote health equity. By providing education, navigation support, and advocacy, CHWs empower immigrants to take control of their health and well-being.

 

 Left Field meeting space is located across the street from PNC Park, Left Field side – literally.

 The address is 116 Federal Street, 4th Floor, Pittsburgh, PA 15212, and is in the same building as North Shore Tavern and Mike’s Beer Bar.

The glass entry doors are situated right between the two establishments. (There is a Left Field sandwich board outside pointing to the doors.)

Learning Objectives

  1. Explain how CHWs can be used to improve refugee/immigrant patients access to care.
  2. -Identify SDOH needs faced by immigrants/refugees and how CHWs can bridge the gap to alleviate those needs.
  3. -Identify resources for providers to address challenges to access care.
  4. -Review currently available CHWs programs and analyze their helpfulness for immigrant and refugee populations.
  5. -Apply knowledge to improve existing or create new CHW programs to support refugee/immigrant patients
