West Penn Hospital STARRT committee’s purpose is to (a) ensure safe and effective patient transitions, (b) transparent communication about readmission opportunity to key stakeholders for readmission rates, cost/incentive impact, targeted population opportunity, process failures, care continuum impact, and (c) identify opportunities to implement change cycles for readmission reduction improvement and/or sustaining performance (proactive and retrospective impact).
